Case Management (Mental Health Services)/Supports Coordination (Early Intervention and Mental Retardation Services) is the hub of services in the Mental Health and Mental Retardation programs. All the services that are available need to be coordinated. That is where case management comes in. The case manager or supports coordinator is the link between the family, the individual and the service provider. Individual's and families' first contact with the system is usually through the MH/MR Case Management Unit. Case managers/supports coordinators: 1) assess the individual's needs and strengths; 2) develop a service plan with input from the individual and the family; 3) arrange and coordinate use of services; 4) monitor the progress of the individual; 5) review the services used to be sure they are appropriate, modifying the plan when necessary; 6) advocate for the individual; 7) work to break down barriers in the community; and 8) seek opportunities for the individual. The goal of case management is to help the individual tap into the services and resources which will enable him/her to reach his/her fullest potential. |
